DAMAGE TO PROPERTY -- An 8-by-4-foot window was broken and one beer was taken from a tap between 2:15 and 2:20 a.m. Sunday at the Paramount Bar, 219 N. Santa Fe; $500 damage, $4 loss.
n Racial remarks were spray-painted on a building at Indian Rock Park, 1500 Gypsum, between midnight and 3 p.m. Saturday; $500 damage.
n A 3-by-5-foot window was broken at 1:42 a.m. Saturday at Laura's Liquor, 1600 E. Iron; $620 damage.
DEPRIVATION OF PROPERTY -- A 2001 Dodge Ram pickup belonging to Lori A. Hoffner, 820 Park, was loaned to a friend Wednesday, and it had not been returned by Saturday; $7,000 loss.

n Popcorn balls chewed through by mice; fresh rodent droppings found on popcorn. Popcorn balls thrown away.

n Popcorn cabinet contained less than 10 rodent droppings with ratio of 1-to-3 fresh-to-old droppings. Droppings cleaned out of cabinet.

n Bartender was drinking from an open cup, then made customer a drink without washing hands first. Instructed.
n Bartender had bare hand contact with ice while making a customer's drink. Instructed.
n Bottles of WL Wellers, Maker's Mark, Wild Turkey 101 and Hennessy Cognac had between one and four fruit flies in each bottle. Alcohol dumped down drain.
n Water lines and ice chute in ice machine had black mold on them. Machine cleaned.

n Bartender handled money, then made customer a drink without washing hands first. Instructed.
n Behind bar, bottle of 3 Olives grape-flavored vodka had piece of plastic broken off in alcohol. Bottle of Jose Cuervo Black had four fruit flies in it. Bottle of Martini and Rossi Rosso and Martini and Rossi Extra Dry each had two fruit flies in them. Bottle of Canadian Dry had white flecks of foreign material in liquid. Contents of all bottles dumped.
n Three-vat sink had no iodine in sanitizing compartment. Iodine added.
n Bartender dipped glasses in sanitizer, then set them on drainboard to dry. Glasses put back in sanitizer for 30 seconds.
n Inside of ice machine contains black mold on food contact surfaces. Ice machine cleaned.

n One fruit fly each found in bottles of Starr African Rum and Old Charter Whiskey. Alcohol dumped.
Napoli's Italian Restaurant, 1935 S. Ohio -- Oct. 23
n Drinking cup setting on pizza make table. Cup removed.
n Dishwasher rubbed his face and chin, then started putting clean dishes away without washing hands first. Instructed.
n Bottle of WL Wellers contained three fruit flies. Contents dumped.
n Large mixing bowl contained dried food debris on inside of bowl, both on the sides and bottom of the bowl, and on the mixer lip. Cleaned.
n Spray bottle of Formula 409 stored with clean skillets. Spray bottle moved.
n Gap was found underneath back door, large enough that a pen can be slid all the way through to the outside.
n Plastic tub of spaghetti in walk-in cooler had large crack along the side; container no longer smooth and easily cleanable.
Wild Wild West, 1540 W. Old Highway 40 -- Oct. 23
n Bartender handled money, used instant hand sanitizer, then made customer a drink without washing hands. Instructed on importance of handwashing.
n Bartender washed hands for total of 8 seconds; soap didn't have time to lather before she finished. Instructed.
n Bartender used dirty common towel, used to open drinks, to dry her hands after handwashing. Instructed.
n Bottle of Martini and Rossi Extra Dry contained two fruit flies. Contents dumped.
n Automatic sanitizing system not hooked up properly. Ecolab was called to fix system and set up bleach solution.
